What Are OWCP Forms?
OWCP forms are official documents required by the Office of Workers’ Compensation Programs (OWCP) to report workplace injuries, illnesses, and request medical care or benefits. Federal employees must complete these forms to initiate or maintain their workers’ compensation claims. Understanding which OWCP form to use is critical—and that’s where we come in.

Common OWCP Forms Explained
✅ Form CA-1: Federal Notice of Traumatic Injury and Claim for Continuation of Pay/Compensation
If you experienced a specific injury from a single event—like slipping and falling on the job—this is the form you’ll need. Form CA-1 is used to report traumatic injuries and request Continuation of Pay (COP) for up to 45 calendar days. It must be submitted as soon as possible, ideally within 30 days of the injury.
✅ Form CA-2: Notice of Occupational Disease and Claim for Compensation
If your condition developed over time due to your work—such as carpal tunnel syndrome or hearing loss—you’ll need to complete Form CA-2. This form is for occupational illnesses and requires detailed medical evidence linking your condition to your job duties.
📝 Other Forms You May Need
- CA-7: Claim for Compensation (used to request wage loss benefits)
- CA-20: Attending Physician’s Report (must be completed by your doctor)
- CA-16: Authorization for Examination and/or Treatment (issued within 7 days of injury for urgent care)
- OWCP-915: Medical Reimbursement Form
